245D Licensed Home and Community Based Services

Basic support services include in home and out of home respite care, personal support, night supervision, individualized home supports, individual community living support, homemaker, adult companion services, and  24-hour emergency assistance. 

In-home support services include supported living services for adults, semi-independent living skills, individualized home supports, independent living skills training, and in-home family support.  

Employment services include- employment support services, employment exploration services, employment development services. 

Housing stabilization services include consultation services, transitioning services, and sustaining services.
